Fees and Insurance

I am in-network with most Anthem commercial plans. I am out-of-network for all other insurance plans. I can provide a superbill for clients who wish to seek out-of-network reimbursement from their insurance provider. Please note that reimbursement is not guaranteed, and it is the client's responsibility to verify out-of-network benefits and submit any required documentation to their insurance company. I offer a limited number of sliding-scale appointments for those who may benefit from reduced-fee services. If you believe you could benefit from a sliding scale option, please reach out.

Out of Network Insurance Reimbursement and Superbills

Upon request, a monthly superbill can be provided for services rendered. A superbill is an itemized receipt that you may submit to your insurance company for possible out-of-network reimbursement.

Please be aware that submitting a superbill requires a mental health diagnosis, which will become part of your clinical record and will be included on the superbill. Reimbursement is determined solely by your insurance company and is not guaranteed.

The practice's involvement is limited to providing the superbill. Clients are responsible for verifying out-of-network benefits, submitting claims, following up with their insurance company, and resolving any reimbursement issues. Amy Cooper LPC, LLC cannot communicate with your insurance company regarding claims, coverage decisions, or reimbursement requests.
